General Information
What does ostomy mean? The term ostomy describes the opening in the body surgically created allowing for the discharge of waste.
What does stoma mean? Stoma refers to the end of the intestine or ureter that can be viewed protruding through the abdominal wall. Stoma in modern Greek is “mouth.”
An ostomy is a surgically created opening on the abdominal wall for waste products to exit the body. It is performed when a medical condition is so severe that an ostomy offers a better alternative. The quality of life is usually much improved for patients with an ostomy, because their previous medical condition was so debilitating.
A colostomy identifies that the opening is from the colon (large intestine). Depending upon which side the opening is (left or right) will sometimes determine what kind of materials are needed to manage the colostomy. Surgically created in the abdominal wall, a colostomy can be temporary or permanent. A temporary colostomy may be required to give a portion of the bowel a chance to rest and heal. When healing has occurred, the colostomy can be reversed and normal bowel function restored. A permanent colostomy may be required when a disease affects the end part of the colon or rectum. Reasons for colostomy surgery may include cancer, diverticulitis, imperforated anus, Hirschsprung's disease, and trauma.
An ileostomy is when the ileum (the lowest part of teh small intestine) is brought to the surface of the abdomen. An ileostomy surgery may be necessary when a disease or injured colon cannot be treated successfully with medicine. The waste that exits from the stoma of an ileostomy can be liquid and sometimes challenging to manage. Reasons for ileostomy surgery may include ulcerative colitis, Crohn's disease, and familial polyposis. There is also a Koch’s pouch, also known as a continent ileostomy. A urostomy is an abdominal opening from the urinary tract which discharges urine. A urostomy also known as urinary diversion is described as a surgically created opening in the abdominal wall through which urine is passed. A urostomy is created when the bladder needs to be removed or is not functioning properly. Some reasons why the bladder may not function properly or need to be removed are: bladder cancer, neurologic dysfunction (bladder), birth defects, and chronic inflammation of the bladder.

Having a colostomy or ileostomy brings some changes when it is time to use the rest room. Waste leaves the body through the surgically created stoma in the abdominal wall instead of the anus. There is no muscle around the stoma, therefore one is not able to control the passage of waste or gas from the body. A urostomy is similar but the stoma is created using a ureter. Waste (feces and urine) are collected in a pouch worn by the individual. Ostomy pouching systems consist of either a two piece or one piece system.
1. A two piece ostomy pouch system includes a skin barrier (flange or wafer) and pouch. The pouch connects to the wafer via a ring that snaps in similar to lids of food containers.
2. A one piece system is a combination of the skin barrier and ostomy pouch in a single unit.
Various other ostomy accessories exist to treat different situations that arise and include:
Curved or convex skin barriers to help improve a seal with some abdomens.
Ostomy belts to provide pouch support and pouch covers for concealment.
Stoma paste which fills in folds or creases in the skin for a good seal with the skin barrier.
Skin wipes and powders which protect the skin from the barrier.
Tapes to help seal the skin barrier when the person may go swimming or take a shower.

Empty your pouch as needed.

Try to get more than a few days wear time from you ostomy appliance. Replace your pouching system every four to seven days. This sometimes includes assessing your stoma and trimming the barrier to size. Replacing pouches for children is approximately every two to three days.

Take extra care for your skin around your stoma and stoma. Be observant of any skin irritation to report to your enterostomal therapy nurse or wound, ostomy and continence nurse.
Treating Skin Irritation
1. Normal color of the stoma is pink to red. Call your stoma nurse or doctor if your stoma:
2. Develops a pale color
3. Becomes dusky or blue
4. Swells
5. Bleeds
If the skin under your ostomy pouch is red, irritated or itchy, you should treat your peristomal skin by:
1. Removing the pouch
2. Clean off the peristomal skin with water and then dry.
3. Dust on karaya powder (natural product with healing properties) to the peristomal skin and blot off.
4. Reattach the ostomy pouching system
5. If skin irritation persists, consult your stoma nurse or doctor.
Psychological Aspects of an Ostomy
A study conducted in the 1990’s by Nick Mihalopoulos, Ph.D. The psychologic impact of ostomy surgery on persons 50 years of age and older. This research article served to address some of the important issues faced by individuals who have ostomy surgery. Below is the abstract:
There have been few studies on the psychologic effects and reactions to ostomy creation in persons older than 50 years. Eighty-nine patients with ostomies (42 female, 47 male) were evaluated to see how ostomy creation affected them. Six instruments were used to make these assessments: Surgery Preparedness Inventory, Surgical Adjustment Scale, Social Interaction Inventory, Self-Esteem Inventory, Geriatric Depression Scale, and Life Satisfaction Inventory-A. In addition to these six instruments, the participants in the study were asked to state their greatest concern. Older men and women showed similar patterns in surgical preparedness, adjustment to operation, and level of self-esteem. Men reported lower satisfaction with life than did women. Men also had more difficulty in social interactions than did women. Twice as many men as women reported mild to moderate depression. Older women reported poorer health status; if unmarried, they reported less satisfaction with life than younger or married women. More of those reporting good current health reported a positive adjustment to ostomy creation than did those who reported poor health. Those who did not feel hindered by their ostomies reported higher scores on well-being than did those who felt hindered. The more time that has passed after operation, the better patients felt. Four major concerns were reported: (1) concern with being able to care for themselves; (2) leakage from the pouch, odor, and gas noises; (3) other health problems; and (4) recurrence of cancer. Good current health and a sense of well-being are important to positive adjustment after ostomy creation. These areas would be well worth monitoring and should be promoted by the caregivers.
